Background: Patients undergoing cardiac surgery are at risk of adverse outcomes, including hospital readmission, postoperative complications, psychological distress, and reduced quality of life. Telenursing-based interventions, including patient education, remote monitoring, and post-discharge follow-up, may improve recovery; however, their effectiveness has not been comprehensively synthesized.
Objectives: This systematic review aimed to evaluate the effectiveness of telenursing‑based interventions on health outcomes following cardiac surgery in adult patients.
Methods: A comprehensive literature search was conducted in PubMed, Scopus, Web of Science, SID, MagIran, and Google Scholar from inception to June 28, 2026. We included interventional studies that investigated telenursing‑based interventions in patients after cardiac surgery and assessed at least one health‑related outcome. Risk of bias was evaluated using the Cochrane RoB 2 tool. A narrative synthesis was undertaken due to substantial heterogeneity.
Results: Of 535 records identified, 11 studies involving 3,262 participants met the inclusion criteria. Outcomes included quality of life, psychological outcomes, self‑care behaviors, physical functioning, pain, hospital readmission, and healthcare utilization. Most studies reported beneficial effects of telenursing interventions, particularly on quality of life, psychological outcomes, self‑care, physical functioning, and reduced readmissions. Two studies had a high risk of bias; nine raised some methodological concerns.
Conclusion: Telenursing-based interventions may complement post-cardiac surgery care by improving patient-centered outcomes and reducing healthcare utilization. However, methodological limitations and heterogeneity warrant further high‑quality studies to confirm these findings and identify optimal implementation strategies.
