<abstract>
Background: Patients undergoing hemodialysis require lifelong adherence to self‑management behaviors to prevent disease complications and improve their overall quality of life. However, maintaining these continuous behaviors poses substantial psychosocial and physical challenges for this patient population.
Objectives: This study aimed to evaluate the effectiveness of the Continuous Care Model (CCM) on self‑management competencies and its specific dimensions among patients undergoing hemodialysis.
Methods: This quasi‑experimental, pretest‑posttest control group study was conducted across the hemodialysis centers of Khoy University of Medical Sciences (Madani and Matin Hemodialysis Centers) from September 2025 to June 2026. A total of 74 eligible patients were selected via convenience sampling and assigned into intervention and control groups (37 patients per group) using non‑random block allocation. The intervention group received the Continuous Care Model structured across four stages—orientation, sensitization, control, and evaluation—over an 8‑week period, while the control group received routine care. Patients’ self‑management was assessed before and after the intervention using the Hemodialysis Self‑Management Instrument (HD‑SMI). Data was analyzed using paired t‑tests, independent t‑tests, and analysis of covariance (ANCOVA) in SPSS software (version 26).
Results: Although the crude post‑test mean difference between groups was not statistically significant, ANCOVA controlling for baseline scores demonstrated a statistically significant improvement. The adjusted post‑intervention mean for total self‑management was significantly higher in the intervention group than in the control group (27.32 vs. 20.11; adjusted mean difference = 7.21, 95% CI [4.24, 10.19], p < 0.001, η² = 0.247). Furthermore, significant improvements were observed in the partnership, self‑care, and emotional management subscales, whereas the difference in the problem‑solving subscale was not statistically significant.
Conclusion: In this quasi‑experimental study, implementation of the Continuous Care Model was associated with significantly higher adjusted scores for overall self‑management, partnership, self‑care, and emotional management at 8 weeks, but not for problem‑solving. These findings should be interpreted cautiously, and further studies are needed to assess the durability of the observed associations.
