Background: Cardiac surgery patients are at risk of adverse outcomes, including hospital readmission, postoperative complications, psychological distress, and reduced quality of life. Telenursing-based interventions, including patient education, remote monitoring, and post-discharge follow-up, may improve recovery; however, their effectiveness in this population has not been comprehensively synthesized.
Objective: This systematic review aimed to evaluate the effectiveness of telenursing-based interventions on health outcomes following cardiovascular surgeries in adult patients.
Methods: A comprehensive literature search was conducted in PubMed, Scopus, Web of Science, SID, MagIran, and Google Scholar from inception to June 28, 2026. We included Interventional studies investigating telenursing-based interventions in patients after cardiac surgery and assessing at least one health-related outcome. Bias risk evaluated using the Cochrane Risk of Bias 2 (RoB 2) tool. A narrative synthesis was performed due to substantial heterogeneity among the included studies.
Results: Of the 535 records identified, 11 studies involving 3,262 participants met the inclusion criteria. Outcomes included quality of life, psychological outcomes, self-care behaviors, physical functioning, pain, hospital readmission, and healthcare utilization. Most studies reported favorable effects of telenursing interventions, particularly on quality of life, psychological outcomes, self-care, physical functioning, and reduction of hospital readmissions. Two studies were judged to have a high risk of bias, whereas nine studies raised some concerns regarding methodological quality.
Conclusion: Telenursing-based interventions may complement post-cardiac surgery care by improving patient-centered outcomes and reducing healthcare utilization. However, methodological limitations and heterogeneity warrant further high-quality studies to confirm these findings and identify optimal implementation strategies.
